How It Works

An assessment will be completed by the Agency to determine eligibility. A personalized care plan will be created with the client. There will be regular visits and feedback provided by the Program Manager and the Personal Support Worker to ensure that your plan is running smoothly.

If you qualify for the Assisted Living Program, there is no fee for the services offered. Lifeline Emergency Response System is available for $20/month, if you qualify.
