Our Client Support team is a dedicated department of Social Workers who act as the system navigators for anyone needing direction and guidance on where to turn for help. Our focus is to support our clients’ journey as they receive and seek additional health and wellness-related services. Our dedicated Social Workers are able to conduct telephone and in-person assessments to identify and address your needs and identify services which can address them. This can include referrals to other organizations.
Our team maintains a strong and collaborative relationship with health care professionals across the continuum of care so that, together, we can work towards one goal: assisting you with the right information and help at the right time!
