Assisted Living
Trained staff provides 15-hour supervised care and 24-hour emergency response to eligible clients in a designated building. Clients are able to benefit from frequent daily visits with services such as medication reminders, meal preps, light cleaning, etc. Assisted Living is designed to provide you with a comprehensive plan of personal support and homemaking assistance. The program is flexible and is designed to complement any assistance you may be already receiving from friends, family members, and peers.
Assisted Living provides services in the following areas:
Personal Support
- Showering/bed bath
- Mouth care
- Hair care
- Preventative skin care
- Assisting in tranfers/positioning
- Dressing/undressing
- Assistance in eating
- Medication reminders
- Assistance with incontenence care
- Reassurance checks
Essential Homemaking
- Light housekeeping
- Laundry
- Meal planning
- Making beds
How It Works
An assessment will be completed by the Agency to determine eligibility. A personalized care plan will be created with the client. There will be regular visits and feedback provided by the Program Manager and the Personal Support Worker to ensure that your plan is running smoothly.
If you qualify for the Assisted Living Program, there is no fee for the services offered. Lifeline Emergency Response System is available for $20/month, if you qualify. |
Locations
The Supportive Housing Program is available at 2 locations in Scarborough: 120 Town Haven Place and 17 Brimley Road.
